My Story log: Tegwen

The story log says that "Tegwen fell in battle." This wasn't actually the case when I completed this. Tegwen lived. She even has dialogue afterwards and Carys makes references to taking Tegwen back to Orr for a picnic after defeating Zaitan. Has anyone completed this mission and have Tegwen die? Loading screen
