I don't want to break anything by trying a big edit, but the next step for norns with the quality of Instinct is called "A Pup's Illness" (level 3). It starts like the others (helping people in Wayfarer Foothills, checking your mail), then it's "Wolf Shaman Gamli needs my assistance." => "Speak to Shaman Gamli at the Wolf Shrine." From this video (http://www.youtube.com/watch?v=uZV7x8z2AAU) just after 10 minutes, you can see that the person who chose Instinct at character creation has a personal story quest named indeed A Pup's Illness.
